It sounds like your right shoulder is coming too far 'out' at times, which can make your angle of attack steep.

Lots of possible variables, but without seeing any pics or video I'd say you either have a right hand grip that is too much in the palm (right wrist not level), and/or are not tracing a straight plane line on the backswing. The focus on the left hand rotation is a way to 'save' the shot but if you don't get it rolling through, your right shoulder jumps 'out' and you get steep (sky shots), or your right shoulder moves well, but you are under plane and you push.

Ball position could also be a factor.

As always - impact fix is the best place to start looking. Is your right wrist bent and level? Is the shaft and left arm in line face on (left arm flying wedge)? and the right forearm in line (down the line - right arm flying wedge)?
